Dr. Richard Gentile, a board certified Facial Plastic Surgeon introduced the “SmartBleph™” short for smart blepharoplasty at the 10^TH International Meeting of Facial Plastic Surgery held in Hollywood Florida April 27-May 2, 2010.

“Smartlifting™, is a proprietary group of procedures introduced by Dr. Gentile in 2008 when the LaserSmartLift™ was introduced as Plastic Surgery’s first laser assisted facelift. In 2008 Dr. Gentile presented these techniques to the American Academy of Facial Plastic & Reconstructive Surgery at their Annual Meeting and to the American Academy of Plastic Surgery at their Annual Meeting. Utilizing the Cynosure SmartLipo™ Nd:YAG laser we are now able to gently separate the layer between the skin and facial muscles without extensive cutting and dissection. The eyelid applications enable the Plastic Surgeons to gently treat the eyelid fat bags without removing them thus reducing surgical trauma and time. Dr. Gentile explained, “Typically in a traditional eyelid surgery incisions are made through skin and muscle frequently denervating the orbicularis oculi muscle. The “Smartlifting™ powered by the Cynosure SmartLipo™ laser, works in eyelid surgery by precisely delivering thousands of tiny laser pulses into the fat bags in the upper and lower eyelid. Each pulse heats a deep column of the fat bag which shrinks its size. These zones of heating also shrink the orbital septum which tends to bulge with age and allow the fat to bulge out. Next the volume deficient area is treated by micro fat grafting to increase the fullness in the depressed areas of the lower eyelids. Laser skin rejuvenation can also be completed with this procedure. Over the next several months following treatment, a natural process called collagenesis results in new, more organized collagen¹. This collagen plumps the skin and results in even greater smoothing in addition to the results obtained by traditional eyelid procedure.”

The FDA has approved Dysport to treat forehead wrinkles and frown lines, making it only the second drug to be approved by the agency as a wrinkle treatment.

Dysport (formerly known as Reloxin) is made from the same active ingredient as Botox (made by Allergan), Clostridium botulinim toxin type A. Botulinum toxins temporarily reduce the appearance of wrinkles by restricting the muscle actions that cause creasing and frown lines.

Like Botox, Dysport is administered via an injection at the site of forehead wrinkles. The FDA based its approval on clinical trials involving about 2,900 people at 80 study sites. Dysport has been used in Europe since the early 90’s
One clinical trial showed 93%-95% of participants noticed an improvement in the appearance of forehead wrinkles within seven days after treatment, and multiple treatments produced effects that lasted for more than 13 months.

The most common side effects of Dysport are nose and throat irritation, headache, pain and skin reaction at the injection site, upper respiratory tract infection, eyelid swelling or drooping, sinus inflammation, and nausea

Sculptra Now FDA Approved For Volumizing Treatment For Face And Nasal Labial Folds

Matt Lauer and The Today Show hosted the Berns triplets from Texas on May 28 in their New York Studio. The triplets were severly burned in a home fire nearly 18 years ago. They were recently treated for mature scar tissue with the Lumenis UltraPulse DeepFX ablative fractional laser. Their interview reports significant improvent in the tone texture and color of the burn scars and is a promising new application for the laser most commonly used for acne scarring.

SmartLipo- Over a year later

We were the first Plastic Surgery practice to introduce SmartLipo to northern Ohio... So what exactly is SmartLipo

Old Lipo: usually under general anesthesia, loose skin remains where fat is sucked out, a 4-to 6-mm tube sucks out solid fat; lots of tissue trauma, scarring where tube enters body; stitches often required, resume work after a week.

SmartLipo: always with local anesthesia, tightened skin from collagen production where lasers are used (visible after three to six months), 1-mm tube fitted with laser melts fat; little trauma, almost no scarring; no stitches needed, resume work after two days.

Why is laser liposuction better than old-school lipo?

Traditional liposuction uses a vacuum with a metal tube attached, which the doctor moves back and forth manually. Because the human hand cannot be very precise, you get a lot of bleeding, swelling and destruction of tissue. But the SmartLipo laser glides through fat like a hot knife through butter, which means less destruction and bruising.

What is the biggest improvement?

Now we can treat a lot more people who, before, would need to have surgery on top of liposuction – either a neck or face lift, because loose skin would get worse from the lipo. Or if they had loose, flabby skin on the arms, they only thing we could do was to cut out the skin.

Now we can use the laser to take out the fat and, as an added benefit, tighten the overlying skin too.

What is laser lipo considered safer?

Because you do not have to go under general anesthesia. There is no surgery that is completely safe, but there have been no serious side effects associated with this procedure. And the recovery times are much faster.

How much fat can one remove in a session?

We do not usually exceed 8 lbs. SmartLipo is for sculpting, not for large volume fat removal.

Where does the liquefied fat go?

The vast majority of fat is either squeezed out of the incisions – like oozing blood – or suctioned out. But because you are suctioning liquid fat, instead of solid, it is not as traumatic. In addition, a very small amount of the fat is absorbed by the body and excreted, probably within 24 hours.

The fat absorbed by the body – are you sure it does it not enter the blood stream?

Blood tests show there is no increase in fatty levels, cholesterol and triglycerides in the blood; there is no increased risk of heart attack or stroke.

How do patients feel after the procedure?

Most people go back to work after two days. They will, however, wear compression bandages for two weeks, on average, compared to up to six weeks with regular lipo.

How long do the results last?

They are permanent. The body has a finite number of fat cells; removed cells do not grow back.

In Memory of Michael E. Debakey, MD

Dr. Gentile joins the Baylor College of Medicine and the host of Baylor Trained Surgeons in mourning the passing of Dr. Michael E. Debakey, the greatest surgeon of the 20th century and Dr. Gentile's first mentor in Surgery who passed on July 11, 2008. Dr. Gentile was a resident in general surgery from 1982-1984 in the Baylor College of Medicine Department of Surgery directed by Dr. Debakey and was one of very few Ohio surgeons to be privileged to have been trained by Dr. Debakey. Dr. Gentile was attracted to Baylor College of Medicine by the principles of excellence, hard work and dedication to patient care advocated by Dr. Debakey. His legacy will be carried on by the surgeons he trained who he introduced to his uncompromising committment to patient care excellence.
